      <image:title>Learn - A strong start - We hosted a very VIP visitor recently: Honourable Magreth Baraka Ezekiel, Member of Parliament representing NGOs, came to see Kafika House.</image:title>
      <image:caption>It was a privilege to show her what we do, and to forge a connection with someone so aligned. We chatted about ways we might collaborate to make high-love care more accessible to the children we serve.</image:caption>

      <image:title>Learn - A strong start - Last month marked a momentous milestone: Dr Bill Brown has given 30 years of service to developing reconstructive and plastic surgery services in Tanzania.</image:title>
      <image:caption>We celebrated the occasion at Selian Lutheran Hospital where some of the many who have benefited from his talent and training had the opportunity to express their gratitude. On behalf of the countless medical practitioners and children whose lives you have touched over these decades - thank you, Dr Brown.</image:caption>

      <image:caption>You’ll meet in person if you come to Kafika House — she’s hosting visitors, sharing her passion for the work we do. She’s helping us turn real human experiences into connections that inspire genuine support, which I’m very excited about.</image:caption>

      <image:title>Learn - The story of Pius - Nearly all of our patients come from just such a setting: a long, bumpy, costly journey to the nearest hospital, and a home environment ill suited to a child who’s just had surgery.</image:title>
      <image:caption>No running water, mud walls and floors, and an outdoor squat toilet – with all the love and care in the world, keeping wounds clean and trying to navigate with legs in plaster casts is simply impossible.</image:caption>

      <image:title>Learn - The story of Pius - Dr Makanza corrected both Pius’s legs in a single surgery.</image:title>
      <image:caption>While on bed rest, he made friends with Sidai, Kelvin and Pisan, delighting in the simple joys of childhood as they healed together. After four weeks, Pius started to walk in his casts: gait training taught him to use his legs anew. Soon, his casts were removed, and our team helped him step into full mobility.</image:caption>

      <image:caption>In a landscape where specialist care is so scarce, he is trained in both general orthopaedics and trauma. <image:caption>It was only when she heard about Kafika House that she learned they could be treated. What struck her was that children who arrived unable to take even a step on their bent-inward feet or bowed legs left walking tall and proud; they were truly being healed.</image:caption>

      <image:caption>He had fallen face-first into a cooking fire during an epileptic seizure. His healing journey took four years and multiple surgeries — including the reconstruction of his nose using a flap from his one remaining arm. Through it all, Vumilia was by his side. <image:title>Learn - All about osteomyelitis - At Kafika House, we see children who have lived with this condition for months or even years.</image:title>
      <image:caption>Many arrive with wounds that refuse to heal; the chronic infection weakens their immune system, which means they are constantly vulnerable to illness, and the inflammation it causes undermines nutrition too.</image:caption>
    </image:image>
    <image:image>
      <image:loc>https://images.squarespace-cdn.com/content/v1/66e0555305b3116fb23d4cb1/d35e337f-ed62-4f25-96c3-1ff392e5f77e/4.jpg</image:loc>
      <image:title>Learn - All about osteomyelitis - In closed fractures, where the bone remains under the skin, splinting can be safe and effective.</image:title>
      <image:caption>In open fractures though – where the broken bone has pierced the skin – the exposure can lead to infection. In cases like these, the wound needs to be properly cleaned and quickly closed, and antibiotics given. When that doesn’t happen, the outcome is often not a good one.</image:caption>
    </image:image>
    <image:image>
      <image:loc>https://images.squarespace-cdn.com/content/v1/66e0555305b3116fb23d4cb1/27344a40-5c1c-4c15-b3cf-4ba5c1010561/7.jpg</image:loc>
      <image:title>Learn - All about osteomyelitis - Traditionally, treatment has involved long courses of intravenous antibiotics – which means patients have to stay in hospital, hooked up to a drip, for weeks or months on end.</image:title>
      <image:caption>70% of Tanzanians live in rural areas, which makes access to sustained rehabilitative programmes nearly impossible. On top of this, 74% of children experience multidimensional poverty.</image:caption>
    </image:image>
    <image:image>
      <image:loc>https://images.squarespace-cdn.com/content/v1/66e0555305b3116fb23d4cb1/6b8ca3c4-65cf-40a0-85b0-bf036c37aa74/6.jpg</image:loc>
      <image:title>Learn - All about osteomyelitis - We have been pioneering a different approach.</image:title>
      <image:caption>Our first step is surgery: we remove the dead bone so that the healthy tissue can recover. Once the source of the infection has been taken out of the equation, the condition becomes much easier to treat.</image:caption>
    </image:image>
    <image:image>
      <image:loc>https://images.squarespace-cdn.com/content/v1/66e0555305b3116fb23d4cb1/3f1c6c57-5d58-477e-b4ff-171c94a04107/3.jpg</image:loc>
      <image:title>Learn - All about osteomyelitis - The next step is antibiotics. Instead of intravenous – administering by drip – we use oral medication in carefully managed doses.</image:title>
      <image:caption>We use a progression of antibiotics, which is to say if one kind doesn’t work, we move on to the next until we find the kind that is correct for the patient.</image:caption>
    </image:image>
    <image:image>
      <image:loc>https://images.squarespace-cdn.com/content/v1/66e0555305b3116fb23d4cb1/f3a5d44b-b6cb-4cf5-8700-9eca794c21ad/1.jpg</image:loc>
      <image:title>Learn - All about osteomyelitis - Over the long term, it’s easy to lose track of how long a child has been on antibiotics, but our team is on hand to ensure that doesn’t happen.</image:title>
      <image:caption>We organise regular clinic check-ups and blood tests, and monitor each child carefully in between to ensure the antibiotics are doing their work.</image:caption>
    </image:image>
    <image:image>
      <image:loc>https://images.squarespace-cdn.com/content/v1/66e0555305b3116fb23d4cb1/96ee9ab1-11fc-4db3-ad4b-6d2987454d1c/9.jpg</image:loc>
      <image:title>Learn - All about osteomyelitis - Thanks to the success of our outreach programme, and the families who become Kafika House ambassadors, our overall average patient age of children born with congenital conditions – like cleft lip and palate, and clubfoot – has lowered.</image:title>
      <image:caption>This shows us that mothers are hearing sooner that help is available, and coming to us to seek it.</image:caption>

      <image:title>Learn - Joseph’s journey - When he was 11, a visitor to his community noticed Joseph, and took him to Kafika House, knowing we’d be able to help - and we did.</image:title>
      <image:caption>His first surgery successfully corrected one foot, and Joseph felt like he’d been given a second chance. After a year in our care, he was discharged, and went back home with a new sense of possibility.</image:caption>

      <image:title>Learn - Joseph’s journey - His second surgery was just as successful, and it was during his recovery that a visitor struck up conversation.</image:title>
      <image:caption>After their chat, she offered to train Joseph at their workshop. He learned to become a bicycle mechanic, then earned a certificate in mechanics through a vocational training programme. Once he was fully healed, he stepped into a new future: he had a job, a place in society, and the means to provide for himself.</image:caption>

      <image:title>Learn - Joseph’s journey - Clubfoot is hereditary, and their first child was born with the same condition as his parents.</image:title>
      <image:caption>Joseph knew what to do: before his son could walk, and before the bullying could start, he brought the boy to Kafika House. By the time he turned three, both feet had been corrected, and now, he runs like any other child. It was no surprise when their daughter followed suit. We welcomed her with joy, and she received the same love and care as her brother and father before her.</image:caption>

      <image:title>Learn - Why do we ask parents to pay? - She began to notice that when treatment or assistive devices (wheelchairs, casts, crutches) were given at no cost, families were much less likely to come for follow-up - or, when they did, that the assistive devices were often broken or hadn’t been cared for.</image:title>
      <image:caption>By contrast, if the family had paid for any part of the process, they remained invested in the outcome; they took care of the devices, and saw the recovery process through. Paying for it made it valuable, even if the cost had been nominal.</image:caption>

      <image:title>Learn - Why do we ask parents to pay? - To be clear - we never let finances become a barrier to treatment; asking for this contribution is not about getting parents to shoulder the burden of the cost.</image:title>
      <image:caption>The families we serve come from backgrounds of scarcity, and their payment usually only covers 2-5% of the total bill. One of the most meaningful contributions we ever received was six eggs, from a mama who had nothing else to offer.</image:caption>

      <image:title>Learn - Mapping the road ahead - Our vision has always been clear: no child in Tanzania should live with a treatable disability.</image:title>
      <image:caption>As we continue working towards this goal, we’re adjusting our approach to make sure that we not only reach more children, but that we're doing so in a sustainable, impactful way. The review was led by our management team. Their recommendations were presented to our Board, who took the time to discuss each one before it was formally approved.</image:caption>

      <image:title>Learn - Mapping the road ahead - We’ve made huge strides in providing excellent care to our children</image:title>
      <image:caption>More than ever, Kafika House is a beacon of hope, transforming the lives of children through surgery, rehabilitation, and ongoing support. Although the global funding crisis has affected our course, we’ve strengthened our operations through improvements in staff capacity, service delivery, and strategic partnerships.</image:caption>

      <image:title>Learn - Mapping the road ahead - We’ve been working closely with government bodies, local leaders, and healthcare providers to ensure that the services we provide are not only impactful but also aligned with national healthcare priorities.</image:title>
      <image:caption>By deepening these relationships, we can continue expanding our reach and improving our service model.</image:caption>

      <image:title>Learn - Mapping the road ahead - The challenges we’ve encountered along our way have invited us to adapt our approach.</image:title>
      <image:caption>Funding constraints have meant scaling down our initial expansion plan - so, we decided to use this as an opportunity to take a more data-informed and strategic approach to our growth. We shifted focus to strengthening our systems, consolidating our efforts, and building a solid foundation for a slower, more deliberate pace of expansion.</image:caption>

      <image:title>Learn - Mapping the road ahead - As we look towards 2027, our priorities are clear: build organisational resilience, leverage the strong partnerships we’ve built to expand our reach.</image:title>
      <image:caption>We will continue to focus on providing high-quality care with a more grounded approach, making sure that every decision we make is backed by data and a clear understanding of the challenges and opportunities ahead.</image:caption>

      <image:title>Learn - Setting futures straight - Often, these children’s hips and ankles are aligned, but their knees bow outwards or bend inwards.</image:title>
      <image:caption>To correct this, surgeons perform a wedge osteotomy. This is a procedure in which a small wedge of bone is removed, bringing the knee back into alignment with the hip and ankle.</image:caption>

      <image:title>Learn - Setting futures straight - Recovery is a journey. After surgery, children spend six weeks in casts.</image:title>
      <image:caption>In the first four weeks, we support them with tailored, bed-based rehabilitation. The next phase focuses on getting them back on their feet, teaching them to walk and exercise while they’re still in their casts.</image:caption>
    </image:image>
    <image:image>

      <image:title>Learn - Setting futures straight - Even once the casts are off, many still walk by swinging their hips instead of bending their knees, not trusting that it’s possible to move normally without pain.</image:title>
      <image:caption>We work with each child on knee flexion and gait training. Soon they discover they can move freely, as though fluorosis had never affected them at all.</image:caption>

      <image:title>Learn - Setting futures straight - Watching a child go from near immobility to walking, running, playing football, and dancing is nothing short of extraordinary.</image:title>

      <image:title>Learn - A journey of first steps - When treated early, the Ponseti method is best: a gentle corrective casting that repositions the feet without surgery.</image:title>
      <image:caption>In Tanzania, stigma surrounding disability means many of our clubfoot patients are older children who have already learned to walk - often in pain - on their inwardly turned feet. Their healing journey is longer, harder, and involves surgery.</image:caption>
    </image:image>
    <image:image>

      <image:title>Learn - A journey of first steps - While the medical term for these cases is 'neglected clubfoot,' we prefer the term 'clubfoot in walking age.'</image:title>
      <image:caption>This language reflects our commitment to empowering parents with understanding and encouragement, ensuring they feel supported rather than blamed. Children with clubfoot in walking age spend an average of 6 months at Kafika House.</image:caption>

      <image:title>Learn - A journey of first steps - Treatment begins with surgery that involves cutting or lengthening tight Achilles tendons, ligaments, and joint capsules to allow for more flexible movement of the foot.</image:title>
      <image:caption>Sometimes, bones may need to be realigned or reshaped too: a tibial osteotomy entails cutting and repositioning the shin bone to improve the foot’s alignment.</image:caption>

      <image:title>Learn - A journey of first steps - After surgery, a cast keeps the foot in position while everything heals.</image:title>
      <image:caption>After 6-8 weeks, physiotherapy begins to help to strengthen muscles, improve range of motion, and increase flexibility - in essence, learning to walk again on two fully functional feet. It’s a long journey, but we’re here to walk it with them, every step of the way.</image:caption>

      <image:title>Learn - Making smiles possible - One of the biggest challenges for these babies is feeding.</image:title>
      <image:caption>Because the cleft makes latching difficult, many of these little ones arrive at Kafika House dehydrated and malnourished. As part of our pre-operative care, we teach their mamas how to express breastmilk and feed their babies using special bottles.</image:caption>

      <image:title>Learn - What we’re made of - Against this backdrop of policies and paperwork, I’ve been thinking a lot about the human architecture of relationships: the intangible elements that aren’t bullet points in a strategic plan, but that bring in the warmth, synergy, love and magic.</image:title>

      <image:caption>I got a photo of a little boy whose one foot seemed to face a different direction than his other. <image:title>Learn - Oh, happy day - Our partners, Global Development Group, host a 'Strength in Partnership' conference every second year, and this time it was in Australia.</image:title>
      <image:caption>Judith, our COO, jetted over to join them and so enjoyed getting to know the wonderful people supporting Kafika House and so many others across the globe. We were so proud to have her representing us - go Judith!</image:caption>

      <image:title>Learn - Our new name has arrived - It meant so much to have your input and inspiration when we were looking for a new name.</image:title>
